The speech–language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreemen

Job Responsibilities

1. Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.

2. Assist the physician, physician assistant, or advanced registered nurse practitioner in evaluating the patient to determine the type of speech or language disorder and the appropriate corrective therapy.

3. Identifies functional speech defects and establishes a plan of care to improve patient’s function.

4. Identifies concerns and implements a plan regarding diet textures, body posture, and special swallowing techniques to ensure safe swallowing at meal times.

5. Provide rehabilitative services for speech and language disorders.

6. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

7. Recommends communication devices/aids as indicated.

8. Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

9. As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.

10. Prepares and submits notes per organization policy. Record activities and findings in the clinical record and to report to the physician, physician assistant, advanced registered nurse practitioner the patient’s reaction to treatment and any changes in the patient’s condition, or when there are deviations from the plan of care.

11. Maintains clinical competency in speech language pathology practice and theory.

12. Identifies patient and family/caregiver needs for other services and refers as appropriate.

13. Instructs patient, family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.

14. May supervise home health aide in procedures directly related to the speech–language therapy plan of care.

15. May be asked to conduct peer review and or clinical record review for like disciplines.

16. May be asked to participate in QAPI programs and functions.

17. Performs other duties as required to facilitate the delivery of speech language pathology services.

Role Essentials

  • Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements:

A. Is licensed as a speech-language pathologist by the State in which the individual furnishes such service; or

B. If the State does not license speech-language pathologists:

1. Must successfully complete 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);

2. Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and

3. Successfully completed an approved national examination in SLP.

  • Complies and maintains current CPR certification
  • ACHA Level II clearance.
  • A minimum of one (1) year's clinical experience in Speech–Language Pathology.

· This role is a part of Humana’s Drivers safety program and therefore requires an individual to have a valid state driver’s license and proof of personal vehicle liability insurance with at least 100.000/300,000/100,000 limits.

  • This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
