Be a part of the Personal Health Coach team -- engage our members to develop lifelong well-being and health.

Humana is seeking a Social Worker to assist identified members in assessing their care needs, assist in planning and implementing interventions to meet those needs, assist in coordinating services, and monitoring and evaluating the case management plan against the member’s personal health care goals.

Role Essentials

  • Current Social Work license in the state in which the individual will be required to practice
  • Minimum of 3-5 years’ experience working as a licensed Social Worker
  • Master’s or Bachelor’s Degree in Social Work or a related field or equivalent education and/or experience
  • Excellent communication skills, both verbal and written
  • Proficient in the use of Microsoft Office products including Word, Excel and Outlook
  • Valid driver’s license and/or dependable transportation necessary
  • This role will require 25% travel to New York City and Long Island with an occasional commute to our Manhattan office
  • This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB
  • Must have the ability to provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required

Role Desirables

  • Health Plan experience
  • Previous experience in care management/discharge planning and/or home health, nursing home inpatient or rehab setting
  • Prior experience with Medicaid or Medicare recipients strongly desired
  • Experience working with geriatric population
  • Knowledge of community health and social service agencies and additional community resources
  • Bilingual is a plus

As part of our hiring process for this opportunity, we will be using an exciting interviewing technology called Montage Voice to enhance our hiring and decision-making ability.
