Clinical Advisor/LCSW - Population Health


Under the direction of the Chief Medical Officer, Faculty Practice Group and the Director of Comprehensive Care Coordination for Population Health Management, the outpatient Clinical Advisor- LCSW Case Manager (CA-LCSW) is a resource to providers, patients, and Care Coordinators, and performs complex care coordination for high risk patients to improve access to psychiatric care and to prevent unnecessary ED admissions and hospitalizations. The LCSW has two main functions to provide social work services that may include assessment, interventions, and recommendations to individual patients and their families: (1) Resource Advisor and (2) Complex Care Coordination the LCSW may provide direct social work services. Qualifications

Current unrestricted LCSW licensure through Board of Behavioral Science in CA
CPR certification is required
Current unrestricted California Driver's License and proof of current auto liability insurance
Master's Degree in Social Work
Demonstrated skill in psychosocial assessment, interventions, and treatment planning with patients and families.
Experienced in short term evidence based treatment approaches such as cognitive behavioral therapy.
Demonstrated work with interdisciplinary teams.
Excellent organizational and time management skills, including the ability to handle multiple conflicting priorities simultaneously.
Demonstrated proficiency in computer skills such as Windows computer function, computer navigation, and ability to use computerized patient care systems, including iPad's and other mobile devices.
Competence in navigating the internet/intranet and use of browser software such as Microsoft Explorer, apps on mobile devices, and products such as Excel, Word, and PowerPoint.
