The Care Connections Project Coord is integral to the delivery of the Novant Health Care Connections health programs, a broad set of activities aimed at improving quality and the experience of care while augmenting value. The Care Connections Project Coord is responsible for project administration and support of the program which includes the collection, management and distribution of project control information required by the program. Drives task management and follow up, ensuring project related information are current and accurately reflect the project state. Responsible for ensuring project leads frequently publish project deliverables, update issues and risks and provide status reports. The position consolidates and maintains project and program plans, status reports, creates and maintains resource plans, coordinates meetings and responsible for building the artifacts used to successfully manage the program. This position will provide project planning and tracking support for care connection activities with a focus on specific project elements that comprise the care connections portfolio. Responsibilities will range from analyses of opportunities to delivery of programs.
